Program Description: Created by Area Agencies on Aging for Area Agencies on Aging. Duett is a web-based tool used by case managers to streamline their client referral workflow to the HCBS or Direct Care Workforce. This is accomplished by digitizing an outdated process of one-off phone calls, emails or faxes. The Duett tool takes a care plan and notifies the direct care workforce of the opportunity, and gives the client and case manager a refined list of service providers.
