Welcome to the Doors to Housing for Older Adults Partnership Action Guide! This guide is intended to help organizations in the aging and homelessness networks form cross-sector partnerships to reduce homelessness among older adults.

In each stage, you will find action-oriented steps to help you pursue partnerships, along with illustrative vignettes based on real experiences and advice from Area Agencies on Aging (AAAs) and Continuums of Care (CoCs) across the country. While these examples focus on AAAs and CoCs, many of the recommended actions are also applicable to form partnerships with other organizations working in the aging and/or homelessness sectors.
