Health Happens At Home Briefing Series
USAging is pleased to introduce a series of briefing papers that explore how community-based care fits into and strengthens the broader health care system. While clinical services delivered in hospitals and other traditional health care settings are important, most of what shapes a person’s health happens in the places where they live, work and connect with others.
By taking a closer look at this reality, this series highlights why community settings and social care professionals are not peripheral to health care, but a foundational part of it.
