What's Happening with the Center of Excellence to Align Health and Social Care?

Since USAging received the award to create a new Center of Excellence to Align Health and Social Care (CoE) this fall, we've started our education work on the role of Community Care Hubs (CCHs) in communities and states, built our team and more. Welcome to our new Director, Paul Cantrell, and Senior Program Manager, Carol Bennett! Maya Op de Beke, Senior Program Manager with our Aging and Disability Business Institute, will continue her work on the CCH National Learning Community.
 
We've also been working closely with ACL and Mission Analytics on the 2023-2024 CCH National Learning Community, in particular the Community Care Hub 101 Learning Series in November and December. To catch up on this four-part overview of the work of CCHs, watch the recordings.
 
Finally, we are developing the Request for Proposals (RFP) for funding for existing and new Community Care Hubs, which will be a critical part of the Center's work. We have been working with ACL to develop the RFP and ensure that it closely and logically aligns with the CCH evaluation process that ACL is undertaking.
