• Nevada Senior Services partners with University Medical Center to facilitate dementia-capable care transitions that emphasize social connection after hospitalization.
• Fairfax Area Agency on Aging maintains its 36-year partnership with Inova Health. These national award winners will share program strategies, present examples and case studies, and answer audience questions.
