Care and Service Coordination Supervisor

The Care and Service Coordination Supervisor is responsible for the direct supervision of Case Managers who provide service for individuals over 60 or individuals under 60 and enrolled in Choices for Care; assists with integration of the Care and Service program with other Age Well programs, and assists with the training, mentoring and evaluation of the Case Managers.
-Assist with organizing and maintaining a data gathering and reporting system as required by federal, state, and local funding sources and for client records
-Maintain ongoing contact with individual Case Managers in order to adequately provide training, supervision and evaluations
-Provide backup assistance to Case Managers as needed
