Nutrition and Wellness Coordinator - Chittenden County
Join the Age Well team to work with Chittenden County partners including senior centers, restaurants and other community-based organizations to create vibrant meal sites, Grab and Go distribution sites and events that interest the older Vermonter population we serve.
Position Responsibilities:
·Oversee and coordinate the operation of multiple meals programs, including traditional, Grab and Go, and Restaurant Ticket Program as well as supporting the Meals on Wheels programming.
·Initiate the creation of new locations with emphasis on expansion of the Restaurant ticket program and coordination of large meal centered events.
·Act as a liaison between Age Well and the community, including clients and their families, health care providers, vendors, Age Well volunteers, and staff.
·Gather and organize data supporting the validity of Age Well’s mission.
·Create clear calendars and schedules including coordination of educational speakers, entertainment and other communications to support and grow Age Well’s programs including the suite of health promotion or wellness programs offered at Age Well.
