Supervisor of Clinical Programs, RN
The Supervisor of Clinical Programs, RN is responsible for providing clinical support and supervision to Age Well’s Care and Service Coordination RNs as well as project-specific Occupational Therapy staff contracted with the agency. In addition to providing supervision and support for day-to-day clinical operations, the Supervisor of Clinical Programs acts as a liaison with local hospital, nursing home, and primary care practice staff throughout Age Well’s service area to strengthen relationships, provide education to community partners about services available to older adults, and to facilitate referrals to Age Well’s programs. Approximately 25% of the role is expected to be spent providing services directly to clients primarily in Addison County and lower Chittenden County, though this portion of the role may lessen over time. This position is strategically focused, and is expected to play a key role in the ongoing development and growth of clinical services at Age Well.
· RN with a minimum BA/BS; BSN Preferred
· Preference for those with clinical experience and/or those with case management experience.
· Mental Health, Aging Services and/or Chronic Conditions experience, a plus.
