Specialized Case Manager - Chittenden County
Specialized Case Managers provide intensive care coordination, assess needs, coordinate, and monitor community services and benefits for individuals over 60 or individuals under 60 with disabilities and enrolled in Choices for Care to promote health, independence, social and economic wellbeing. Specialized Case Managers serve individuals who are vulnerable or at-risk, or who have medical, social, or emotional challenges that add complexity to their care needs. In order to meet the needs of these individuals, the Specialized Case Manager needs to have and maintain an understanding of our community's diverse resources, services and benefits. The Specialized Case Manager visits clients in the community to offer practical assistance, person-centered guidance, and support. This is a field service position with a degree of complexity and challenge. Particular attention is paid to the most vulnerable and at-risk individuals in the community at large.
