Specialized Case Manager
Specialized Case Managers provide intensive care coordination to older adults who are vulnerable or at risk, or who have medical, social, or emotional challenges that add complexity to their care needs. In order to meet the needs of these older adults, the Specialized Case Manager needs to have and maintain an understanding of our community's diverse resources, services and benefits for people over 60 in order to promote health, independence, social and economic wellbeing. The Specialized Case Manager visits clients in the community to offer practical assistance, person-centered guidance, and support. This is a field service position with a degree of complexity and challenge. Particular attention is paid to the most vulnerable and at-risk older adults in the community at large.
We are filling two positions. One position will serve Chittenden and Franklin Counties and the other will serve Chittenden and Addison Counties.
