Transitions Liaison Nurse

The Transitions Liaison Nurse is a new, grant-funded position that will work with hospital and nursing home administrators, social workers, care managers, and admissions/discharge planning staff to assist with developing and coordinating safe discharge plans for clients transitioning from inpatient or residential care to the community. The Transitions Liaison Nurse will consult on discharges as well as complete transitions of care visits with clients after they have transitioned back to a home setting.

The successful candidate will be an RN with a minimum of a BA/BS degree. BSN Preferred. Work with fostering relationships and interdisciplinary collaborations preferred.  Clinical experience and/or case management experience preferred; Mental Health, Aging Services and/or Chronic Conditions experience a plus.
