New American Team - Community Health Worker - Chittenden
The New American Team - Community Health Worker (CHW) will augment the goals of the primary Case Manager (CM), by carrying out components of the care plan as designated by the CM. The CHW is responsible for helping older adults and their families navigate and access community services and other resources and adopt healthy behaviors. They provide social support and informal counseling, advocate for individuals and their needs, and provide services such as home visits and medication screenings. The CHW provides an assessment of need and eligibility for public benefits, medication reconciliation, and nutrition screening. Provide assessment for falls risk, conduct nutrition screening and evidence-based medication reconciliation for seniors. Assess the need for public benefits and provide application assistance.

The successful candidate for this position will have knowledge of the needs, challenges, and potential of older New Americans living in our four county service area.
The ability to speak and write clearly in English and Nepali and the ability to speak multiple languages, and/or to use translators, when necessary, is required.
