Community Health Worker
The Community Health Worker (CHW) position augments the goals of the primary Care & Service Coordinator (CSC) by carrying out components of the care plan as designated by the CSC. The CHW is responsible for helping older New American adults and their families navigate and access community services and other resources and adopt healthy behaviors. The successful candidate will have knowledge of the needs, challenges, and potential of older New Americans as well as an understanding of resources available.

The successful candidate for this position will have knowledge of the needs, challenges, and potential of older New Americans living in our four county service area.
The ability to speak and write clearly in English and Nepali and the ability to speak multiple languages, and/or to use translators, when necessary, is required.
