Case Manager - Chittenden County

Our Case Managers assess needs and coordinate and monitor community services and benefits for people over 60 to promote health, independence, social and economic well-being. This is a field position of some complexity, with particular attention paid to the most needy and at-risk individuals in the community at large. This position is also expected to work in the office on a regular basis. Mileage reimbursement provided.
