The Administration for Community Living (ACL), in collaboration with the Center of Excellence to Align Health and Social Care (powered by USAging’s Aging and Disability Business Institute), invites community-based organizations serving as community care hubs (CCHs) or interested in learning more about the community care hub model to participate in an upcoming four-part Community Care Hub 101 Learning Series. The series will take place on four Thursdays in mid-November through December 2023.
A community care hub is a community-focused entity that organizes and supports a network of community-based organizations providing services to address health-related social needs. It centralizes administrative functions and operational infrastructure, including but not limited to, contracting with health care organizations, payment operations, management of referrals, service delivery fidelity and compliance, technology, information security, data collection, and reporting.
Each webinar will focus on the foundational elements of serving as a community care hub. The webinars will showcase examples from leading community care hubs as well as resources available to support community-based organizations in forming, operationalizing, and launching a hub in their community.
