Each year, The John A. Hartford Foundation Business Innovation Award recognizes community-based organizations and networks that are improving health outcomes and quality of life for older adults and/or persons with disabilities through health care contracting.

During this webinar, the 2023 recipients of The John A. Hartford Business Innovation Award, Western New York Integrated Care Collaborative, and the runners-up, Central Ohio Area Agency on Aging and Region IV Area Agency on Aging in Michigan, will share information about their award-winning innovative programs. Attendees will learn their strategies for developing their contracts and partnerships, their challenges and the outcomes they achieved.
