Addressing Medicare Readmission Rates Through Partnerships with Health Care

To encourage hospitals to provide higher quality care and achieve better health outcomes, Medicare reduces payments to hospitals that have high rates of readmission, infections and injuries. AAAs and other CBOs are developing partnerships with health care entities to help reduce these rates while addressing the social determinants of health and creating new funding streams.

The latest round of readmission penalties announced by CMS this month affected more than 2,500 of the 3,129 hospitals evaluated under this program. Find out if a hospital in your community is facing readmission penalties this year.
