n4a Member Receives New Federal Pilot Funding to Integrate Social and Medical Care

The Denver Regional Council of Governments (DRCOG) was awarded $4.5 million in CMS funding for a federal pilot program designed to foster integration between clinical and community service providers.

DRCOG's Area Agency on Aging will be leading coordination efforts on the program in their area. 
 
The Accountable Health Communities (AHC) model will test whether identifying and addressing the health-related social needs of Medicare and Medicaid beneficiaries' through screening, referral and community navigation services will improve care, impact health care costs and reduce health care utilization.
 
As one of only two CBOs to be awarded the funding directly, DRCOG will use the $4.5 million to serve as a hub for 16 regional partners addressing health-related social needs including housing instability, food insecurity, domestic violence and transportation.
