Leveraging CBO Networks to Improve Health Outcomes in California

A new publication from n4a's Aging and Disability Institute describes how Partners in Care Foundation built a network of community-based organizations from scratch to provide services to 50,000 individuals who have chronic health conditions.

When Partners in Care Foundation (Partners), a nonprofit community-based organization (CBO) in California, was approached by one of the state's largest health plans about a partnership that would provide chronic disease management and community-level case management to 50,000 Californians, it rose to the challenge. Despite there not being a model for such a network and only having six months to get up and running, Partners saw tremendous potential in this partnership opportunity.

The network Partners created is now responsible for 30 health care contracts and has been able to achieve measurable results such as 50 percent improvement in medication adherence and a 24 percent reduction in 30-day hospital readmission rates.
