Deadline Monday for The John A. Hartford Foundation 2018 Business Innovation Award

There's still time to submit your nomination for The John A. Hartford Foundation 2018 Business Innovation Award! Submit nominations by Monday, June 11 if you're a CBO, or know of one, that has a dynamic partnership with a health care entity that is improving the lives of older adults, people with disabilities and their families.

Now in its third year, The John A. Hartford Foundation Business Innovation Award showcases the achievements of CBOs that are successfully partnering with health care entities to improve the lives of older adults, people with disabilities and their families. The nomination period ends Monday, June 11.
 
Previous winners of this prestigious award include the Southern Maine Agency on Aging (SMAA) and the Virginia Area Agencies on Aging—Caring for the Commonwealth (VAAACares).
 
The award will be presented at n4a's 43rd Annual Conference & Tradeshow in Chicago, IL in July and includes a plaque and monetary award of up to $5,000. Nominees will be notified of the results prior to the Conference.
