Virginia AAA Partnership Model Provides Comprehensive Care to Dual Eligibles
A new Aging Today article, developed as part of a series of case studies from n4a's Aging and Disability Business Institute, profiles the work of Virginia Area Agencies on Aging-Caring for the Commonwealth (VAAACares), recipient of the 2017 The John A. Hartford Foundations Business Innovation Award.
The article highlights how the VAAACares initiative provides comprehensive care coordination, care transitions and additional services to support the health and well-being of its enrollees.
The partnership model, launched in October 2015, is referred to a as a "one-stop-shop" for referrals, billing, reporting, data analytics, training and quality assurance, and has served over 7,000 Medicaid and dually eligible beneficiaries to date.
