Now that Medicare's open enrollment period has ended, many agencies are gearing up to provide information to older adults in their communities about preventive visits that help older adults stay healthy.
These visits also provide a great opportunity to help older adults to stay up-to-date on important medical screenings and create a wellness plan with their health care professionals. Order FREE copies of our Wellness Wisdom fact sheet and other #OurBestShot resources to share in your agency's Medicare educational sessions and services!
Wellness Wisdom: Schedule Your Medicare Preventative Visits Every Year provides an overview of the two kinds of preventive visits covered by Medicare—the "Welcome to Medicare" visit and Annual Wellness Visit—and a checklist of important items to prepare for your visit. The fact sheet also includes a list of recommended vaccines for older adults and if/how they are covered by Medicare.
Your agency can also order FREE copies of a “truth teller” fact sheet that explains the facts about vaccines and a quick guide (also available in Spanish) highlighting the symptoms of common infectious diseases.
A community workshop kit provides your agency with all the necessary tools for using these resources in educational workshops for older adults and caregivers in your community, including a Leader's Guide, slide presentation, evaluation forms and much more.
Visit the #OurBestShot campaign page to view these resources and order FREE COPIES at no cost to you or your agency while supplies last.
