Trailblazing CBOs Focus of Article Series on Integrated Care Success

A new collection of articles profiling innovative CBOs that are part of n4a's Trailblazer Learning Collaborative (TLC) is now available on the Aging and Disability Business Institute website.
CBOs across the geographic and service spectrum are positioning themselves for integrated care collaborations with a variety of health care payers and providers. The new Aging Today supplement, a publication of the American Society on Aging, features four TLC organizations sharing lessons learned and best-practices around how to approach and implement integrated care strategies. These groups are the “leading-edge” of the Aging and Disability Networks, the first group of CBOs to tackle next-generation health care partnership issues.
