How These AAAs Landed Hospital or MCO Care Transitions Contracts

Yesterday, n4a hosted a member-only webinar showcasing five AAAs across the country that are funded by hospitals or MCOs to do care transitions work. 

Attendees heard directly from these AAAs on how they earned the support from private payers and had the opportunity to ask questions of the presenters. If you missed the webinar, you can view the recording now.
 
Presenters included: 
  • Orion Bell, CEO, CICOA Aging & In-Home Solutions, Inc., Indianapolis, IN. Orion discussed how his agency's history of relationships with the health care community in Indianapolis and asking hospitals “how can we help who you serve?” helped his AAA move forward when opportunities arose to partner with hospitals.
  • Dr. Steven R. Counsell, Director, IU Geriatrics, Indiana University School of Medicine, Indianapolis, IN. Steve, an ally of Orion's AAA, provided a health care perspective and shared tips for the Aging Network when partnering with hospitals and physicians.  
  • Kristen Griffis, Executive Director, Elder Options, Gainesville, FL. Kristen stressed the importance of good data, good relationships and learning how to speak the language of health care to build a partnership between AAAs and health care systems. 
  • Dr. Yolanda Lovato, Planning and Resource Director, Aging & Long Term Care of Eastern Washington, Spokane, WA. Yolanda offered the perspective of a AAA contracting with an MCO and discussed her agency's commitment to changing their branding to better market themselves to the health care community.
  • Bill Cooper, Transitions Program Coordinator, Bluegrass Area Agency on Aging & Independent Living, Lexington, KY. Bill emphasized the importance of using the language best understood by the entity you are working to partner with in your business and marketing plan. He also stressed always placing your data upfront, to show how you can benefit the partnership.
  • Julie Olson-Tobias, Director of Client Services, REAL Services, Inc., South Bend, IN. Julie highlighted the steps her AAA took to land a contract with a hospital, starting with relationship building with the discharge planning managers, then working your way up the chain of command. Julie also advised stressing your AAA's role as experts in community resources.
